The purpose of this survey is to collect data to be presented to our elected officials in Washington, DC on Dec. 6, 2005.
 
We need as many veterans as possible to take this survey in a very short amount of time so if you have Veteran friends not on the computer please either fill out for them or have them go to their library and fill it out.

How far must you commute to the nearest VA facility?

Have you ever used a VA facility for healthcare?

If Yes, how would you rate the care you were given?

Did you find the medical facilities to be clean and neat?

How were you treated by the medical staff?

Are there discrepancies in your records vs. what you told the Drs.?

If yes, did you request that these discrepancies be corrected?

If yes, were they corrected?

Did any of your VA doctors suggest you file a claim for disability?

If yes, did they offer to assist you in filing the claim?

Considering your disabilities, are you able to navigate your local facility with ease?

How were you treated by the service officer who handled your claim?

What service organization represented you?

Would you recommend your service officer to another Veteran?

How much power do you feel the VA has over the service officers?

What year did you file your first claim with the DVA?

Do you have a service connected disability?

If yes, how long did it take to get your disability approved?

How many times was it denied before it was approved?

Do you feel that time was reasonable?

When you finally get through all the bureaucratic doors and get to speak to a VA representative:

What was their attitude? 

Did they listen carefully?

What were the results?   

How many times have you made it to the BVA?   

If the BVA agreed with your claim why do you think it was denied 2, 3, 4, or more times by the VA before you got heard by the board?   

Have any medical services at your local VA been cut?

If so, would the civilian facilities accept the VA vouchers?

Do you think Veterans are on any priority list with our government?
